Alright so I have 2009 Sierra Crew Cab, with 5" RCX lift (4.5" rear blocks). Sometimes when I'm on dirt roads or really loose gravel and I'm spinning the tires and my foot is to the floor. My rear end will start to shudder and shake, this only happens once and awhile. Other than that the truck drives perfectly fine, no vibrations, nothing. I'm thinking it's an axle wrap issue? but can't confirm. If it is, what are my potential solutions, will add a leaf help fix this?

Traction bars plus replacing those blocks with a lift leaf pack would help (traction bars would help even with blocks, plenty of ppl run this) . Or get traction blocks from OneUpOffroad..

That is a good idea with the Go pro, thanks. I'm thinking my leaf springs are really bagged out because when I go from a dead stop my rear end sags a lot, more then I think it should. Also when I stand on the rear bumper and jump up and down, there seems to be quite a bit of travel in the rear suspension.

Yes I have twice, the first time I checked the U-Bolts were really loose, nothing really loose on the front end just a couple of odd ones, the second time, barely. So I think I'm good but I could check everything again.
