You don't have to. But if you plan on pulling a trailer or carrying anything remotely heavy in the bed, it's a good idea.
I've been thinking about selling my Mcgaughy's 4/6, and I have their c notch kit that hasn't been installed yet. If you're really interested in lowering.

How close to flush with the fenders are your wheels and tires?

Squatting a little with a loaded down 5x8 uhaul. I got rid of the lift hangers and an 7” in the rear now. Also took the 24’s off for a while and threw on some 2014 oem wheels with a 275/45 tire. Helpers going on next weekend...

I put lift hangers on my 09 ext cab. At a 7in drop out back the rear axle was maybe an inch from the frame (no notch). When I put air in my assist bags it would raise the rear about an inch anyways.

I had hangars and a flip for 6”. I put OEM style hangars back on. I have an underbed notch though.
